From the moment you schedule your first appointment with Houston Methodist, our team partners with you to ensure you receive the highest level of comfort and care.

Important Documents for the First Visit
When you call to schedule an appointment, you will speak with a clinical coordinator who will help you get started. To make sure your appointment is focused on your or your loved one’s condition, your coordinator will send you important documents to fill out in advance. These documents include:
- Medical history questionnaire — This form covers medical information, history of primary care physicians and information on any specialists already seen.
- Release of medical records — This form grants us permission to request relevant scans, procedures or medical reports that may help us understand your loved one’s condition as it relates to potential causes of dementia or Alzheimer’s disease.
- Caregiver questionnaire — This form is for a current caregiver to provide answers about your loved one’s memory, judgment, problem-solving abilities, social interaction, hobbies and personal care.
What to Expect at the First Appointment
We encourage caregivers to join all appointments. The first appointment will last one to two hours. You will meet with one of our world-renowned specialists, and you may ask as many questions as necessary to help you understand your options.
The doctor will conduct tests and schedule additional examinations. These results will help our team understand your loved one’s current cognitive status and overall health as we define next steps:
Cognitive Tests
- Memory – Evaluates both long-term and short-term memory.
- Executive function – Evaluates the ability to plan or manage basic daily behaviors and understand how things work, both visually and functionally.
- Neurological function – Evaluates reflexes, balance and gait.
Labs
Your doctor may recommend bloodwork to help with analysis and to help identify any underlying conditions contributing to neurological disease. Test samples help identify inflammation, electrolyte imbalances, vitamin imbalances, heart or vascular disease, anemia, bacterial infections, thyroid imbalances, calcium deficiency and other conditions.

- Sleep study
We will coordinate a meeting with one of our compassionate and experienced social workers who provide valuable tools and assistance for every step of your journey.
Though a thorough diagnosis might take longer, you will leave your first visit with a plan to move forward. Your doctor will answer your questions and define clear, actionable next steps.
The Second Appointment
After we review the results from the tests and examinations, you and your loved one will undergo a follow-up visit with your initial doctor. This doctor will ask follow-up questions and provide more specific information.
You will leave your loved one’s second appointment with a detailed treatment plan, prescriptions for any necessary medications and follow-up appointments booked. Our team is here to guide you through next steps.
