At Houston Methodist, our Health Information Department is dedicated to maintaining a high level of privacy and confidentiality with all patient records. We keep all health information private and secure in accordance with federal and state regulations.
- Online Form: Complete the "Online (Docusign) Patient Access Form" by clicking the link associated with the hospital. Once you have completed the form, "submit" to send to the selected location.
- PDF Form: Print and complete the Patient Access Form PDF and send via mail, fax or email at one of our locations listed below.
- MyChart: Request a copy of your records through MyChart: https://mychart.houstonmethodist.org
- Phone: Call one of our office locations at one of the phone numbers listed below.
- Not the Patient: If you are not the patient but would like to request medical records of a patient, download the Authorization for Use and Disclosure of Health Information form and mail, fax or email it to the Houston Methodist facility where services were rendered. The form must be filled out by the patient or the patient's guardian/legal representative.
Choose an Option Below to Request an Amendment to Your Protected Health Information (PHI)/Medical Records
As a Houston Methodist patient, you have the right to request an amendment to your PHI/medical record when you identify a mistake. To request an amendment, either:
- Print and complete the Request for Amendment form and submit either by email or mail per the instructions on the form

MyChart Proxy Access
Houston Methodist offers the ability to access a loved one's MyChart under specific circumstances through a process called Proxy Access. To obtain access, either log into your MyChart app and request access or use the Houston Methodist online proxy application process below. If you request proxy access within your MyChart app, the request will be sent directly to your loved on in MyChart for approval. Note: If you want access to a minor patient, you must use the option below.
Online Proxy Application Process
- If you are a parent of a minor child, you can obtain access to their MyChart as part of your own by completing an online proxy form here.
- If you have guardianship, power of attorney, executor of the state, or other legal paperwork that legally allows you to have access to an adult's medical record without authorization, you may complete the same form here and someone from Houston Methodist will contact you to obtain the additional paperwork.
- If you want to access any other adult's MyChart record, you can obtain access by completing this online proxy form which will be routed to the patient for approval. Once the patient approves, the proxy link will be established and you may access their MyChart.
